INDIANAPOLIS – The Indiana Family and Social Services Administration Friday announced that software used by managed care entity CareSource experienced a security breach that exposed protected health information of some Indiana Medicaid members. The names, addresses, Social Security numbers, dates of birth, gender, medical conditions, diagnoses, medications, allergies, health conditions, member ID and plan name of 212,193 members of Indiana Medicaid who are part of a CareSource managed care plan may have been exposed in the breach. The City of La Porte and 39 North Conservancy District announced' Friday that agreement had been reached on a Memorandum of Understanding (MOU) providing for the City to purchase and operate the district's water and sewer system as part of the city's annexation effort north of the city. A 62-year-old driver from Lowell was transported to the hospital with life-threatening injuries Thursday after a crash involving an SUV and a dump truck northwest of Lowell, in West Creek Township. At about 6:17 a.m., Lake County Sheriff’s Department police officers were dispatched to U.S. 41 and 157th Avenue for a report of a traffic accident. A preliminary investigation indicates the driver of the SUV was headed north on U.S. 41 when she attempted to turn left or make a U-turn at 157th Avenue. On Monday Sept. 4, City Hall and non-emergency City Operations will be closed for observance of Labor Day. The transit system will also be closed for the holiday. Two people were arrested after a crash involving a semi on I-80/94 early Tuesday. At approximately 3:45 a.m., troopers were dispatched to I-80/94 eastbound lanes near the 7.7 mile-marker for a crash involving a semi and a car. Preliminary investigation shows that a 2015 Ford Taurus side-swiped a 2001 Freightliner, causing the semi to crash into the median wall. The semi continued to travel over the wall and overturned, coming to a rest on its driver’s side. The semi also struck some construction equipment that was being operated by two construction workers. Press release by City of La Porte: Residents with RTS trash totes will finally be rid of them by the end of next month, according to Mayor Tom Dermody. On Saturday, Sept. 23, the City, along with third-party contractor LakeShore Recycling, will collect the totes from residents throughout the city.
